The pouches are all from 5.11 Tactical.com except the two small ones on her front right hip and the one that holds the First Aid Spray at her back left.
The first pouch is the "6.6 Padded Pouch" for $24 on 5.11. For the two side pouches, I used two "Disposable Glove Pouch" in black from 5.11, but you can also get them on Amazon. (When dealing with lots of blood it's good to have rubber gloves...)
I also used a "Reebow Gear EDC Tactical Bag"/(Admin Molle Tool Bag/Organizer, black), and took it apart to add another layer to the 6.6 padded pouch. (Will do a walk-through of that in another post.) This is not something that's needed for the outfit, in fact it will make it a whole lot easier to do without this, but I did it because I wanted to.

I combined all of these together to get the pouch that she carries on the belt at her back:
I added a handle at the top and quick-release buckles. You can get quick-release buckles on Amazon in a 25mm 10 pack or 20mm 10 pack for $10ish. The ones on the above pouch are 25mm. The sliding straps that came with the "6.6 Padded Pouch" are used to attach to the pistol belt. Here are a few in-game shots/pics I used for reference:
The second attachment on her belt is the 5.11 "Flash Bang Pouch" for $19 off Amazon. (Most of the stuff from 5.11 can be found on Amazon for slightly less.)
The two smaller pouches pictured here^ on her right hip, I borrowed from my Starbuck (BSG) outfit. https://www.anovos.com/ They used to have a utility belt for sale that included all the pouches. Not sure where you can get the belt now but it's possible to find the pouches on a police/military site.

They velcro on to the pistol belt and fit well but are not completely necessary.
The holster I mentioned in the last post. I used the same one I had for Carter and they can be found at many military surplus stores or websites. I got mine from code3tactical.com but I don't think that website exists anymore. The holster attaches to the pistol belt in front of the flash bang pouch and behind the two small pouches (or wherever is comfortable).
Finally, the First Aid Spray attachment. In the above pictures, you can see that the cans are only held by a strap around them (from what I can tell). There are a bunch of ways to do this I suppose but I like to use what I have if I can to save materials/money and put something together creatively. If I can't then I'll buy. I actually used some leftover canvas material that I used to modify Carter's vest, plus velcro, plus part of the strap that was on the harness I bought for Jill, plus the part of a guitar strap that sits on your shoulder...

There's probably a thing that exists out there for something like this but, honestly, I didn't even look because my mind kind of jumbled up this contraption. But it turned out really well and fits and is comfortable and everything. So do what works for you. I'll probably do a more detailed walk-through on this as well but for now, in a nutshell, I slid the strap from the harness through the guitar shoulder strap. I wrapped the strap around until there was no more excess and marked where to sew on the canvas with velcro. I sewed on the velcro the the canvas, and the canvas to the strap. Then I sewed the strap to itself so it couldn't move. Then I made velcro straps that attached to the pistol belt.
I tried all of these completed pouches on the pistol belt at the same time to check for weight, balance etc. I made some adjustments like moving the molle straps up or down so that the pouch wouldn't lean away from my back. It's comfortable, light and I can still sit down easily! This worked for me, you can tweak it to what will work for you.
